Youth Risk Behavior Survey Consent Form

Dear Parent/Guardian,

Our school is taking part in the Youth Risk Behavior Survey (YRBS), a survey designed to assess and monitor the health behaviors of youth. The survey aims to gather important data to help identify areas of concern, inform programs and policies, and promote the well-being of our students. This is in partnership with our Community Service Board and the United Way of Southwest Virginia. Please carefully review the following information.

Youth Risk Behavior Survey Details

The purpose of this survey is to assess physical and mental health and wellbeing in youth to identify areas of strengths and challenges and inform our programs and policies. The survey covers a range of topics including but not limited to: positive relationships, mental health, and substance use. The survey will take approximately 30 minutes to complete. The survey will be conducted in school between February and March of 2024.

Confidentiality and Anonymity

The survey has been designed to protect your child’s privacy. No identifying information (e.g., your child’s name) will be collected. The only potential risk is that some students might find certain questions to be sensitive. Students may skip any questions they do not wish to answer. In addition, students may stop taking the survey at any point without penalty. No action will be taken against the school, you, or your child if your child does not take the survey.

Data collected from this survey will be shared with Emory & Henry College to analyze and make sense of the trends in students’ behavior. The purpose of this data sharing agreement is to assist the schools and health professionals in our district in making informed decisions about programming for our students. Data may be used by Emory & Henry College for future research. There is no risk in your child’s identifying information being shared.
